* How can I arrange for home care services?
A phone call to our office, or completion of the Request Health Services form will put you in touch with one of our Supervisors. Each Supervisor has several years of home care experience. After a brief conversation with you, we will determine which home care options are the most appropriate for your needs
* Who typically pays for home care services?
Home Care Services can be paid for directly by the Patient, by family members or through private insurance companies. Prior to providing services, Avondale Home Care will screen your benefit plan and confirm your level of coverage with you.
* How long has Avondale Home Care been serving the community?

* Is Avondale Home Care licensed?
Avondale Home Care is a home care agency licensed in the State of Georgia and the Department of Human Resources
* How does the Agency select and train its employees?
Avondale Home Care carefully screens and selects its employees. All applicants are tested, have experience, and are certified or licensed. Each member of our personnel must pass a criminal background check before being assigned to a patient. Applicants are fully bonded and insured. References are required.
* What kind of communication is kept on Patients & Relatives?
Each Patient has a "Care Plan" which is a list of specific tasks requested by Patient/Representative during the admission process.. The Care Plan is tailored to the specific Patient's needs and is updated at each visit at least every 90 days or as needed. The Patient and/or family member receives a copy of the Care Plan and the original is kept in the Patient's file in our office.
* How are emergencies handled?
A Patient Care Coordinator is on call 24-hours per day, 7 days per week. A Nurse can be reached to arrange for immediate assistance, in case of an emergency. |
